Patient Health Alliance MedicAlert ID Program

Patient Health Alliance (PHA), in partnership with MedicAlert, is helping individuals diagnosed with NMOSD access an important tool for safety and emergency preparedness.

Through generous funding from Alexion, AstraZeneca Rare Disease, eligible individuals diagnosed with NMOSD can receive a MedicAlert bracelet or necklace of their choice, a one-year MedicAlert subscription, and two MedicAlert ID cards at no cost.

MedicAlert identification can help communicate important health information in an emergency, particularly when an individual may be unable to communicate for themselves.

This program is one of the practical ways PHA works to connect the NMOSD community with tools that support safety, preparedness, and informed care.

Getting your MedicAlert ID through PHA is simple:

Complete the PHA request form.

Tell us a little about yourself and confirm your NMOSD diagnosis through our online form.

Choose your MedicAlert ID.

Visit MedicAlert.org to explore available bracelet and necklace options. Choose the style you prefer and, if selecting a bracelet, measure your wrist to determine the appropriate size.

Connect with the PHA Advocacy Team.

A member of our Advocacy Team will contact you to complete your enrollment, confirm your ID selection and sizing, and help you through the next steps to finalize your order.

Receive your MedicAlert ID.

Once your enrollment and order are complete, your MedicAlert ID will typically arrive in less than one week.

Through the PHA MedicAlert ID Program, individuals diagnosed with NMOSD receive at no cost:

A MedicAlert bracelet or necklace of their choice
A one-year MedicAlert subscription
Two MedicAlert ID cards

Already Have a MedicAlert ID from GJCF?

If you previously received your MedicAlert ID through The Guthy-Jackson Charitable Foundation (GJCF), you do not need to request a new ID. PHA can renew your MedicAlert subscription through this program. Complete the request form and let us know that you are a previous program participant.
